Patrick Fisher
Patrick Fisher is a functional safety engineer at Provenance Consulting with experience in the industrial and refining industries with proven results in process hazard analysis (PHA)/layer of protection analysis (LOPA) facilitation and training, safety instrumented function design, and IPL management.
As a PHA facilitator, he helps companies meet the PHA, MOC, PSSR, and PSI requirements of the OSHA PSM standard, and helps manage the results of the PHA/LOPA results.
His background includes publications on risk analysis, independent protection layer management, and leading indicators of process safety.
